Who do you work with?
We provide emergency, short term and long term placements for care leavers and young adults with mild to moderate and complex mental health, drugs or alcohol misuse and clients that have experienced sexual or criminal exploitation.
We provide specialist support for single parents with one child who have experienced domestic abuse.
What is the maximum age?
There is no maximum age, however the minimum age is 18 years old.
How do I make a referral?
Download the initial enquiry form and email the form to referrals@childandfamilysocialcare.com. Then we will have an initial telephone consultation to discuss vacancies and appropriateness of the referral with regard to the needs of the young person.
When will I know the outcome of the referral?
Referrals received before 2pm are reviewed by the daily panel at 4pm and a decision is made on the same day, we will then send you an invitation for a further consultation within 24 hrs. If the young person is offered a place they will be notified within 48 hours. Once a place has been offered we will arrange for the young person to visit the accommodation and meet their support worker, read their tenancy agreement and sign the house rules.
What happens next?
We use a person centred approach and we will work with a potential client and both evaluate if we are the right fit.
A planning of care meeting will be held with your key worker and your referrer to complete the risk assessment and write a care and support plan.
Once a place is offered to you, you will have an induction and sign your contract outlining the expectations of the placement. If you are offered a place you will then be asked to sign a tenancy agreement.
How long can I stay?
There is no minimum or maximum period that you can stay in the accommodation.
Who will I live with?
You will live to 2 to 4 other residents in a house of shared occupancy. The residents share the communal areas kitchen, bathroom, lounge and garden.
We integrate young people in emergency, short term and long term placements through one to one sessions and group activities to support young people with change and transitions.
What happens if I breach my tenancy agreement?
You will receive a verbal warning, a written warning and a final written warning. This will depend on the nature of the breach and in some circumstances an immediate eviction notice can be served, we try to work with you to avoid eviction.
What support is available?
All the residents are provided with one to one support for 15 hours a week. However we can provide additional hours if required to meet your individual needs.
We support you to budget, access training, education, employment and housing.
Key work sessions
The key worker will monitored and review the progress of the care plan and meet with the young person to support them to achieve the objectives in their care plan.
What specialist services can I access?
We provide specialist domestic abuse and sexual exploitation services to young people.
When will I see my family and friends?
We can discuss family and friends time as part of the planning of care meeting to ensure if that if this is appropriate as part of your care and support plan.
Can my family and friends stay overnight?
Unfortunately we are not able to accommodate overnight guests due to the complex needs of the residents.

We can discuss staying overnight with family and friends as part of the planning of care meeting to ensure if that if this is appropriate as part of your care and support plan.
What happens after I leave the accommodation?
We can support you to contact housing to find accommodation if your placement has not been terminated. We work closely with the service user and supporting agencies to facilitate a successful move on into local authority accommodation or the private rented sector. We provide least three months of outreach/floating support to ensure the young person is settled in their accommodation.
Staff
The staff are DBS checked and received ongoing mandatory training.
